How will the form get completed ?

The reference number/venue/date/owners name and product info/description of problem  will be provided by the meet and greet team and the visitor. 

If it is very busy, the product info may need to be completed by the visitor/repairer when the visitor is with the repairer. 

The meet and greet team will ensure the visitor signs the disclaimer form on the rear. 

The meet and greet team will then direct the visitor with their item and form to the appropriate repairer.  

The repairer should complete the repairer's comments when the repair is completed, or it has been determined that a repair cannot be completed.

The repairer should give the form back to the visit who then takes it back to the meet and greet desk, unless it is an electrical item - in which case, the visitor needs to be directed to the PAT testing station.

The PAT tester will need to complete the last box on the page and aask the visitor to return the form to the meet and greet desk.
